After being in shelter on and off, with only a social security card and an expired out of state ID

After being in shelter on and off, with only a social security card and an expired out of state ID, Jay had given up on obtaining the rest of his documents in order to apply for housing. After some persistence in finding secondary documentation, our case managers were able to submit a birth certificate application and quickly received his birth certificate. From there, Jay and our case managers were able to acquire a Utah ID. Jay then applied for subsidized housing and he will shortly be moving out of shelter and back into a place of his own!
