This referral form is for Family Interim Non-Congregate Children's Housing (FINCH).

The eligibility requirements for FINCH are:

• A family of 4 if all family members are over the age of one year.

OR

• A family of 5 if one family member is an infant.
• Priority placement is given to with the youngest children, and/or children currently enrolled in Granite School District.
